How does Lamicet 50mg Tablet work?

Lamicet 50 controls seizures or fits by decreasing the abnormal and excessive activity of the nerve cells in the brain.

Quick Suggestions:

• It may cause dizziness and sleepiness. Do not drive or do anything that requires mental focus until you know how it affects you.
• It may cause headaches. Drink plenty of water and take a suitable painkiller. Inform your doctor if it does not go away.
• It may cause a skin rash. Inform your doctor immediately if you notice a rash, itching, and peeling of skin.
• Talk to your doctor if you notice sudden mood changes or develop suicidal thoughts.
• Do not stop taking the medication suddenly without talking to your doctor as it may increase the frequency of seizures.

Lamotrigine reduces excessive brain nerve activity, controlling seizures by:

1. Stabilizing nerve cell electrical activity
2. Suppressing the release of glutamate, an excitatory neurotransmitter

It's commonly used in pediatric epilepsy due to its safety profile. Lamotrigine has no risk of psychological or physical dependence and no potential for abuse.

Pharmacology

The exact mechanism of action of lamotrigine is not fully understood. It is believed to work by inhibiting voltage-sensitive sodium channels, stabilizing neuronal membranes, and modulating the release of presynaptic excitatory neurotransmitters. Lamotrigine blocks sodium currents by selectively binding to inactive sodium channels, which suppresses the release of the excitatory amino acid glutamate. This mechanism is thought to be similar in both its anticonvulsant and bipolar disorder treatments. Additionally, lamotrigine may bind to various receptors, which could explain its efficacy in treating neuropathic pain.

Dosage of Lamicet 50mg Tablet

Adult Dose for Epilepsy:

• Initial: 25 mg once daily for 2 weeks, then 50 mg once daily for 2 weeks
• Maintenance: Increase by 50-100 mg every 1-2 weeks
• Usual maintenance: 100-200 mg daily, single or divided doses
• Maximum: Up to 500 mg daily in some cases

Adult Dose for Bipolar Disorder:

• Initial: 25 mg once daily for 2 weeks, then 50 mg once daily for 2 weeks
• Maintenance: Increase weekly to 200 mg daily
• Maximum: 200 mg daily

Child Dose for Epilepsy (>12 years):

• Similar to adult dosing

Child Dose for Epilepsy (<12 years):

• With valproate: Start at 0.15 mg/kg daily, increase gradually
• Maintenance: 1-5 mg/kg daily in 1-2 divided doses

Dosing adjustments are required for hepatic impairment and when used with other medications like valproate or enzyme-inducing antiepileptics. Caution is advised in renal impairment.

Missed Dose & overdose instructions

• If you miss a dose of Lamotrigine, take the missed dose as soon as you remember. If it’s almost time for your next dose, skip the missed dose. Do not double your dose to make up for the missed dose.
• Seek emergency medical treatment or contact the doctor in case of an overdose.

Administration of Lamicet 50mg Tablet

Swallow it as a whole with water; do not crush, break, or chew it. 

Before taking Lamicet 50, inform your doctor if you are taking any of the following medicines:

• Medicines used to manage epilepsy and mental health problems (Ex. oxcarbazepine, phenobarbital, primidone, phenytoin, valproate)
• Medicines used for HIV management (Ex. lopinavir, ritonavir, atazanavir, ritonavir)
• Medicines used to manage pregnancy (Ex. ethinylestradiol, levonorgestrel)
• Rifampin (used for bacterial infections like tuberculosis)

Pregnancy & Lactation

Lamotrigine is classified as Pregnancy Category C. It is excreted in human milk, and due to the potential for serious adverse reactions in nursing infants, a decision should be made to either discontinue breastfeeding or the medication, considering the drug's importance to the mother.

Precautions & Warnings

1. Inform your doctor if you're allergic to Lamotrigine or any other medications.
2. Consult your doctor before taking Lamotrigine if you're pregnant, planning pregnancy, or breastfeeding. It may harm the unborn baby and affect the child's cognitive abilities later in life.

6. Don't stop Lamotrigine without your doctor's advice, as it may cause life-threatening seizures.
7. Lamotrigine can cause severe or life-threatening skin rashes, especially in children, those on high starting doses, or those taking valproic acid. Report any skin rashes, hives, blistering, peeling, or mouth/eye sores to your doctor immediately.
8. Lamotrigine causes drowsiness and dizziness. Only drive if you're alert.
9. Avoid alcohol while taking Lamotrigine, as it can increase dizziness and sleepiness.
